Role and Responsibilities
As a Clinical Pharmacist at Clinix Health Group, your role will involve leading and monitoring clinical pharmacy services. Your main responsibility will be managing patient medication therapy to improve outcomes. This includes daily ward rounds, reviewing medication profiles, and making necessary pharmaceutical interventions.
You will also collaborate with healthcare professionals, such as doctors, microbiologists, nurses, and infection control staff, to ensure safe and effective medication use. Additionally, you will play a key role in Antimicrobial Stewardship, ensuring medications are used appropriately for patients.
Key Responsibilities Include:
- Daily Ward Rounds & Medication Reviews:
Conduct medication reviews, monitor responses, and intervene as needed to optimize therapeutic outcomes. - Antimicrobial Stewardship:
Monitor antimicrobial use, analyze data to identify misuse, and implement interventions to optimize drug utilization. - Collaborative Practice:
Work with doctors, microbiologists, nursing staff, and others to provide evidence-based drug information and medication therapy management. - Professional Development & Training:
Mentor pharmacy staff, foster skills development, and stay updated on clinical knowledge through continuous education. - Medication Safety:
Monitor safety, identify risks, and address medication therapy challenges. Participate in adverse event reporting and interventions. - Reporting and Documentation:
Maintain accurate patient records, document interventions, and prepare reports for the Clinix Clinical Risk Committee. - Protocol Development:
Assist in creating and implementing drug administration protocols to manage medication risks. - Continuous Improvement:
Engage in ongoing improvements to ensure optimal patient outcomes.
Qualifications & Experience
To be considered for this role, the following qualifications and experience are required:
- Pharm Degree and a Post Graduate Qualification in Clinical Pharmacy.
- Active Registration with the South African Pharmacy Council.
- Five (5) years of experience as a pharmacist (post internship), with at least two (2) years in Clinical Pharmacy, including experience in Antibiotic Stewardship, Pharmacovigilance, and Rational Medicine Use.
- Proficiency in Microsoft 365 applications, including Excel and PowerPoint.
- Excellent written and verbal communication skills, with the ability to work effectively in a team setting and collaborate with multidisciplinary healthcare providers.
- Additional experience with the Blue Bird programme and SAP would be advantageous.
